As shown in the creatine safety page, long-term creatine use does not significantly affect clinical markers of health in athletes. Studies have not found clinically significant deviations from normal values in renal, hepatic, cardiac or muscle function.

However, additional water should be taken as some of the weight gained will be through water retention in the muscle. The creatine monohydrate molecule may bind to water molecules, dehydrating you. Thus, it's highly recommended to drink additional water and stay hydrated when using creatine.
